Student requires proof of private violin instruction...does anyone have a template?

Edited: May 9, 2020, 10:25 AM · I have a high school student who needs me to write a letter for his orchestra class that certifies that he has been taking violin lessons with me for at least 9 weeks. Is there a template anyone has that I can use? I'm just not sure how to go about writing such a letter.

May 9, 2020, 11:07 AM · I think just a simple letter that states "______________ has been taking 30(?) minute lessons, once a week(?) since (date).
May 9, 2020, 11:34 AM · I agree with Jim. Just an ordinary letter on your own personal stationery would be fine. If you can look up the teacher's email address on the school's web page, so much the better (email does not carry COVID-19). Make sure to include your phone number.
